About Us
Our company works in partnership with the state agency, NJ Division of Developmental Disabilities. We perform case management services for adults with developmental disabilities. We have been providing this service, called Support Coordination, since 2015. We have extensive experience and a strong reputation.
We are seeking a full-time employee to work in the Monmouth, Mercer, Middlesex, Ocean and Union county areas. Prior experience in case management is not necessary. We provide our employees with a comprehensive training program. This is a hybrid position working from home and some field work meeting people in their homes or in the community. Our work involves helping people find support services to meet their needs and ultimately achieve their life goals.
Responsibilities
- Collaborate with the individuals served, their families, natural supports, service providers, physicians and other community resources.
- Arrange and coordinate services including assistance in developing community natural supports. Actively participate in the development of community based contacts, networking and information gathering within the assigned geographic area. Remain in contact with the supported individual and their family to ensure continued satisfaction with services.
Skills & Requirements
- Competent meeting facilitation skills required. Good communication skills and follow-up a must. All paperwork and reports must be completed in a professional manner, including accurate grammar and spelling. This job has extensive written documentation requirements.
- Employees must be able to work from home and be willing to spend up to 30% of their time in the field. A reliable car and the ability to travel to client locations in Mercer, Monmouth, Ocean, and Middlesex county is required.
- Applicants MUST have experience of 1 year working with adults with intellectual/ developmental disabilities or some equivalent work. Having a family member with an intellectual/ developmental disability is considered a substitute for work experience.
