Job Summary

Responsibilities
- Assist residents or patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs such as blood pressure, pulse, and temperature regularly
- Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance
- Support residents with meal preparation, feeding, and ensuring proper nutrition tailored to individual needs
- Maintain cleanliness of living spaces by performing light housekeeping tasks such as cleaning and laundry
- Observe and document changes in residents’ conditions, behaviors, or symptoms to inform healthcare teams
- Use specialized equipment like Hoyer lifts or resident’s
to assist with heavy lifting or transfers safely - Follow all safety protocols including CPR and First Aid procedures to respond effectively during emergencies
- Collaborate with healthcare professionals, social workers, and family members to develop and implement personalized care plans
- Engage residents in social activities or companionship to promote mental well-being and reduce feelings of isolation
Experience
- Prior experience working in assisted living facilities, nursing homes, group homes, or home & community care environments is highly desirable
- Knowledge of dementia care, Alzheimer’s care, memory care, hospice care, and working with individuals with developmental disabilities or autism is beneficial
- Demonstrated ability to perform patient observation and monitor vital signs accurately
- Experience with medication administration and following detailed care plans according to healthcare regulations such as HIPAA
- Familiarity with using mobility aids like Hoyer lifts or resident lifts for safe patient transfers
- Background in providing long-term care or home health services that emphasize compassionate caregiving and patient safety
- Certification in CPR and First Aid is preferred; ongoing training in dementia or memory care is a plus
