Job Overview
We are seeking a dedicated and knowledgeable Registered Nurse to join our non-medical private duty home health agency. The RN would visit clients in their homes or place of residence to conduct Assessments and create Care Plans. The ideal candidate will play a crucial role in coordinating client care and ensuring the client receives the highest quality of care from our caregiver staff. This position requires a strong understanding of health conditions that may impact the geriatric population and the ability to work effectively within a team environment.
Responsibilities
- Conduct assessments on client to evaluate for needs of assistance with activities of daily living
- Evaluate living environment for safety hazards for the client
- Provide dementia support for to client and families
- Maintain compliance with HIPAA regulations to protect patient privacy and confidentiality.
- Collaborate with other healthcare professionals and other Agency RNs to develop individualized care plans.
- Monitor client progress and adjust care plans as needed based on ongoing assessments.
- Document all patient interactions, assessments, and care provided in electronic health records.
Experience
The ideal candidate will possess the following skills and knowledge:
- Strong understanding of physiology and anatomy relevant to client care.
- Experience in geriatric nursing practices.
- Knowledge of HIPAA regulations and commitment to maintaining client confidentiality.
- Familiarity with dementia care techniques and strategies.
- Proficiency in assessing clients to evaluate for needed assistance with activities of daily living
- Ability to work effectively within a home health agency while prioritizing client needs.

Pay: $35.00 - $40.00 per hour

Education:
- Bachelor's (Required)
Experience:
- Home care: 2 years (Preferred)
