Advantage Family Outreach and Foster Care (AFOFC) is a private, non-profit foster care network that is Title IV-E approved and CARF-accredited. For over 25 years, AFOFC has provided high-quality care and services to youth in foster care. Our Independent Living Group Homes are designed to prepare adolescents for self-sufficiency by teaching essential life skills to youth who are aging out of the foster care system.

Job Overview

The Group Home Case Manager is a key member of the Independent Living program, responsible for supporting and guiding youth as they transition into adulthood. This includes developing service plans, teaching life skills, coordinating care with service team members, and ensuring documentation and regulatory compliance. The GH Case Manager works closely with youth, group home staff, and other support teams to promote stability, independence, and long-term success for residents.

Key Responsibilities

Case Management and Service Planning

  • Conduct initial assessments by reviewing documentation, meeting with youth, and consulting with their service team (e.g., GAL, caseworker, probation officer).
  • Assist the LSW in developing, implementing, and updating individualized service plans for each resident in compliance with OAC and CARF standards.
  • Coordinate and lead quarterly service review meetings with youth, agency staff, service providers, and other service team members.
  • Serve as a consistent advocate for youth, ensuring clear communication between all involved agencies, schools, employers, and family supports.
  • Maintain accurate, timely documentation of assessments, service plans, and other required records related to each youth's care and progress.

Independent Living Skills Development

  • Provide hands-on, age appropriate instruction and modeling of essential life skills, including budgeting and financial literacy, education support and academic planning, job readiness and resume development, household management (cooking, cleaning, hygiene), navigating healthcare and health education, and building and maintaining healthy relationships and boundaries.
  • Facilitate individual and group-based skill building sessions tailored to each youth's developmental level and goals.

Youth Support and Supervision

  • Ensure youth's daily needs are consistently met and that they are engaged in structured, skills-based programming.
  • Provide direct care and supervision, as needed, both onsite and during offsite independent living activities.
  • Support and coordinate attendance at school, work, medical appointments, court hearings, visitations, and extracurricular activities.
  • Foster a safe, respectful, and supportive environment that encourages youth self-reliance, accountability, and personal growth.

Communication & Compliance

  • Maintain consistent communication with group home staff, clinical services, and other members of each youth's treatment support team.
  • Accurately complete all required documentation and ensure records are up to date and compliant with agency policies and regulatory standards.
  • Support ongoing audits and quarterly file reviews related to resident care and case documentation.
  • Uphold strict confidentiality and ensure all communications align with legal, ethical, and professional standards.
  • Participate in required staff meetings, supervision, and training sessions to support continuous learning and team collaboration.

Required Skills & Qualifications

  • At least 21 years of age
  • High school diploma or GED
  • Valid OH driver’s license, reliable transportation, and ability to meet agency driver requirements
  • Must be able to pass varying background checks, included but not limited to BCI and FBI
  • Minimum of 1-3 years of experience in behavioral health case management or a related field.
  • Strong interpersonal and communication skills, with the ability to work effectively with clients, families, and interdisciplinary teams.
  • Ability to work independently, manage a caseload, and prioritize tasks in a fast-paced environment.
  • Experience working with youth and a basic understanding of independent living skills
  • Must be able to be trained in and comprehend trauma informed care and how to deal with complex childhood trauma

Preferred Skills & Qualifications

  • Familiarity with OAC and CARF standards related to group home care.
  • Bachelor degree in social work, psychology, sociology, criminal justice, or a related field.

Schedule & Location:

  • 8 hour shift, occasional evening hours may be required
  • Monday to Friday
  • Location: In-person work at our Mansfield or surrounding area group homes is a regular and essential part of the role.

Job Type – Full-Time, Regular, Exempt

Pay Range - $18.00-$24.00 per hour, commensurate with education and experience
