Overview
Advantage Behavioral Healthcare has a career opportunity for an Intensive In-Home Team Lead Licensed or Associate Licensed professional to work with the child and adolescent population. This position provides home and community based treatment services to children and adolescents with behavioral health and/or mental health disorders. The Intensive In-Home Qualified Professional ensures the completion and implementation of the Person Centered Plan (PCP), Psychosocial Interventions, and Crisis Intervention while working with a multi-disciplinary team to meet the needs of the individuals and families.
Duties and Responsibilities:
- Coordinates the initial assessment and reassessment of the member’s clinical needs.
- Completes the initial development and ongoing revision of the Person Centered Plan (PCP) and ensuring its implementation.
- Provide planned treatment intervention for individual member per the PCP.
- Coordinates the initial and ongoing assessment activities.
- Convenes the Child and Family Team meetings (CFT) for the purpose of updating the Person Centered Plan.
- Providing and coordinating behavioral health services and other interventions for the individual or other family members with other licensed professionals and Child and Family Team members.
- Consulting with identified medical (for example, primary care and psychiatric) and nonmedical (for example, DSS, school, DJJDP) providers, engaging community and natural supports, and including their input in the Person Centered Planning process.
- Ensuring linking, arranging and coordination to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
- Monitoring and documenting the Outcomes and Status of all members assigned
Required Education & Experience
- A master’s degree in a human service field and 1 year of full-time, post-graduate degree accumulated MH/DD/SUD experience with the population served.
- OR a bachelor’s degree in a human service field and 2 years of full-time, post-bachelor's degree accumulated MH/DD/SUD experience with the population served.
- OR a bachelor’s degree in a field other than human services and 4 years of full-time, post-bachelor's degree accumulated MH/DD/SUD experience with the population served,
- Strong clinical knowledge of the treatment/service needs of the members served.
- Skills in assessing strengths and needs, Person Centered Plan development, and clinical service implementation and documentation.
Skills
The successful candidate should possess the following skills:
- Strong understanding of Cognitive Behavioral Therapy principles.
- Experience with Behavioral Therapy modalities.
- Effective case management skills, including documentation and reporting.
- Ability to perform crisis intervention effectively.
- Competence in behavior management strategies tailored for diverse populations.
- Familiarity with behavioral health concepts and practices.
- Proficient in Motivational Interviewing methods to enhance client motivation.
Job Type: Full-time
Pay: $21.00 - $26.00 per hour
