Advanced Specialty RX Pharmacy is a dedicated Long‑Term Care pharmacy providing high‑quality medication services to skilled nursing facilities, assisted living communities, and other LTC partners. We support our facilities through accurate dispensing, exceptional service, and reliable monthly medication cycle exchanges. We are seeking a dependable Pharmacy Technician – Medication Swapper to join our team and serve as an on‑site representative for our LTC partners.
Position Summary:
The Medication Swapper is responsible for visiting assigned LTC facilities to complete monthly medication exchanges (“med swaps”), ensuring residents receive accurate and up‑to‑date medication cycles. This role requires strong attention to detail, professionalism, and the ability to work independently while representing the pharmacy on‑site.
Key Responsibilities:
- Travel to assigned LTC facilities to perform monthly medication cycle exchanges
- Retrieve expiring or discontinued medications and replace them with updated cycle‑fill medications
- Verify medication quantities, labels, and packaging for accuracy before and during swaps
- Communicate with facility nursing staff regarding discrepancies, missing items, or urgent medication needs
- Maintain organized medication carts, bins, and storage areas during each visit
- Document completed swaps, returns, and any issues encountered during the visit
- Ensure proper handling, storage, and transport of medications in compliance with state and federal regulations
- Return unused or expired medications to the pharmacy following established procedures
- Assist pharmacy staff with cycle‑fill preparation, packaging, and staging when not on facility routes
- Uphold professionalism and serve as a positive representative of the pharmacy at all LTC partner locations
- Follow all HIPAA, Board of Pharmacy, and LTC regulatory requirements
Qualifications:
- High school diploma or equivalent required
- Active Pharmacy Technician License/Registration in Michigan
- National certification (CPhT) preferred
- Prior LTC pharmacy experience is a plus
- Valid driver’s license and reliable transportation required
- Strong attention to detail and accuracy
- Ability to work independently and manage time efficiently
- Professional communication and customer‑service skills
- Comfortable with repetitive tasks and physical activity (lifting medication totes, pushing carts, etc.)
Schedule:
Monday - Friday
9am - 530pm
