Role Description
This is a part-time hybrid role for a Long Term Care Physician, based in Columbia, MO, with some opportunities for remote work. The selected candidate will provide patient-centered medical care, including diagnosing, treating, and managing chronic and acute medical conditions for residents of long-term care facilities. Responsibilities include contributing to care planning, collaborating with multidisciplinary teams, conducting patient assessments, and ensuring adherence to clinical best practices and regulatory guidelines.
Qualifications
- Strong clinical knowledge and experience in primary care, geriatric medicine, and chronic disease management
- Ability to perform thorough patient assessments and create effective care plans
- Proficiency in electronic health records (EHR) systems
- Strong communication and interpersonal skills for multidisciplinary collaboration
- Board certification or eligibility in Geriatrics, Internal Medicine, or Family Medicine
- Active and unrestricted medical license in the state of Missouri
