Licensed Practical Nurse (LPN) - Home Care with Trach Experience

LPN and RN Roles at a Glance

Home Care:

• Assist with ADLs, meal prep, hydration, and setup of medical equipment (e.g., commode).
• Organize medications into pill boxes and create Medication Administration Record (MAR).
• Educate patient on medication schedule and purpose.
• Set up a medication calendar and coordinate follow-up doctor appointments.
• Fill out appointment and medication logs; contact home health for care needs assessment.
• Explore potential eligibility for VA/respite care.

Hospital:

• Ensure patient discharge readiness, including removal of personal items and reconciliation of medication records.
• Consult with care team about obtaining new or refilled medications (onsite or patient’s preferred pharmacy).
• Transport patient home (stop at pharmacy if needed).

Requirements:

• LPN State License
• BLS Certification
• Annual Physical Exam
• TB Test or Chest X-ray
• Yearly FLU vaccine
• Fully COVID-19 Vaccinated
• Minimum one year of experience required
