Work Arrangement: Onsite at IPA Member Practices (Hybrid work possible)
Location:
• 1 NP for Los Angeles County – Koreatown Area
• 1 NP for Orange County – Various Sites
Position Summary:
We are seeking dedicated and mission-driven Clinical Physician Assistants (PAs) to support our growing network of Primary Care Providers (PCPs) who operate under Full Risk Medicare Advantage Value-Based Care (VBC) Contracts. These PAs will serve as clinical extensions of our MSO, delivering Annual Wellness Visits (AWVs), preventive screenings, chronic care assessments, and driving the closure of care gaps and quality performance measures (e.g., HEDIS, STARS, RAF-related coding).
Additionally, the PA will support the training and upskilling of other mid-level providers on compliant documentation and VBC workflows while directly engaging in clinical transformation efforts at the practice level.
Key Responsibilities:
Clinical Care Delivery & Patient Engagement
• Perform Annual Wellness Visits (AWVs), preventive screenings, and chronic care management visits in close coordination with IPA PCPs.
• Identify and deliver disease detection screenings aligned with patient risk factors and clinical guidelines (e.g., A1C, fall risk, colorectal cancer, depression).
• Close patient-specific clinical care gaps to improve HEDIS and STAR measure performance, following MSO protocols and workflows.
• Deliver empathetic, culturally sensitive, and patient-centered care with a focus on senior and high-risk Medicare populations.
• Educate patients and families on health conditions, medications, screenings, and preventive care, reinforcing self-management principles.
• Collaborate with Care Managers, Quality Leads, and Risk Adjustment Teams from the MSO to coordinate and optimize patient care.
VBC Quality Performance & Risk Adjustment
• Utilize evidence-based guidelines and MSO documentation standards to accurately and compliantly capture clinical diagnoses during visits.
• Adhere to MEAT criteria and CMS HCC coding protocols to support RAF (Risk Adjustment Factor) accuracy.
• Participate in clinical documentation audits and chart reviews to improve diagnosis recapture and reduce documentation errors.
• Leverage point-of-care tools and pre-visit planning documentation to support effective, targeted visits.
• Engage with MSO Risk Adjustment and Clinical Documentation Improvement (CDI) teams to ensure appropriate chronic condition capture.
Mentorship & Support
• Serve as a clinical resource for medical assistants, care coordinators, and support staff within the practice to improve team-based performance.
• Partner with the MSO’s Clinical Operations team to improve provider alignment with quality and risk adjustment expectations.
Operational Support & Clinical Transformation
• Act as a clinical liaison between the MSO and assigned practices, assisting with VBC transformation projects and operational improvements.
• Support practice workflow changes related to EHR optimization, clinical documentation workflows, and team-based huddles.
• Participate in quality committee meetings, VBC workshops, and performance improvement initiatives.
• Submit clinical reports, training feedback, and performance dashboards as required by MSO leadership and CMS guidelines.
Qualifications:
Education & Licensure
• Graduate of an accredited Physician Assistant program with a current PA-C certification.
• Active and unrestricted Physician Assistant license in the state of California.
• Valid DEA license and NPI number.
• BLS certification required; ACLS preferred.
Experience
• 2 years of experience as a practicing PA in a primary care, geriatrics, or internal medicine setting.
• Prior experience in Medicare Advantage and/or VBC environments strongly preferred.
• Demonstrated proficiency in conducting AWVs, documentation of chronic conditions, and care gap closure strategies.
• Experience training or mentoring mid-level providers is a plus.
Skills and Competencies
• Strong interpersonal, coaching, and communication skills.
• Excellent EMR documentation, clinical reasoning, and coding awareness.
• Familiarity with HEDIS, STARS, CMS HCC Coding, and RAF scoring.
• Self-starter who can function both independently and as part of a care team.
• Ability to organize, prioritize, and deliver outcomes in a fast-paced environment.
• Culturally competent, especially when working with multilingual or senior patient populations.
Working Conditions & Travel:
• Travel to multiple assigned IPA practice locations within the geographic region required.
• Some hybrid flexibility for administrative tasks, documentation review, and MSO-led training.
• Occasional evening or weekend availability may be requested for outreach or quality improvement efforts.

Salary : $140,000 - $170,000
