Position: PRN Certified Registered Nurse Practitioner (CRNP) – Hospice
Location: Pittsburgh and surrounding areas (homes, assisted living facilities, skilled nursing, occasional hospitals)

About the Role
We are seeking a compassionate and flexible PRN CRNP to join our hospice team in the Pittsburgh area. This position is designed for a provider who values independence, enjoys a flexible schedule, and is committed to providing high-quality care to patients and families at the end of life.
The primary responsibility will be conducting hospice recertification visits:
- What is a recertification? Hospice patients must be evaluated every 60–90 days (per Medicare guidelines) to ensure they continue to meet eligibility criteria for hospice services. Recertification visits document ongoing terminal decline and are essential to maintaining appropriate services for patients and families.
CRNP visits may also be required for:
- Admissions for patients who have previously been on hospice, completed their first two benefit periods, and are now being re-admitted.
- Transitions back to home health, if a patient improves, is discharged from hospice, and needs a face-to-face evaluation for referral to home health services.
Key Details
- Flexible scheduling: Recertification visits are assigned on Day 1 of the new benefit period. CRNPs have 30 days to complete the visit and may schedule with patients/families at their convenience.
- In-person visits only (required by CMS).
- Settings: Homes, assisted living facilities, skilled nursing facilities, and occasionally hospitals.
- Support & resources: Company-supplied tablet, full EMR access, and collaborative support from hospice RNs and leadership.
- Training: Education provided on hospice eligibility guidelines, documentation standards, and team support if not already fluent.
Responsibilities
- Perform timely hospice face-to-face (F2F) recertification visits.
- Assess patient's current clinical condition, disease trajectory, and continued hospice eligibility.
- Document thoroughly in the EMR and communicate findings to the hospice team.
- Provide compassionate education to patients and families about the hospice philosophy of care.
- Collaborate with the hospice interdisciplinary team as needed.
Qualifications
- Current PA CRNP license in good standing.
- Hospice and/or palliative care experience preferred (training available).
- Strong communication and interpersonal skills.
- Ability to work independently and manage own schedule.
- Compassionate and patient-centered approach.
- Reliable transportation to travel to patient homes/facilities.
