Job Title: Clinical Transition Nurse
Position Overview:
Advanced Hospice is seeking an experienced and resilient RN Clinical Liaison to join our fast-growing team. This role is essential in providing high-quality hospice care by ensuring seamless communication and education among patients, families, and healthcare facilities.
Location:
Pittsburgh, PA
Schedule:
Monday to Friday with some rotating on-call responsibilities.
Travel Requirements:
This position requires 100% travel to patient homes, facilities, and hospitals as necessary.
Responsibilities:
- Conduct hospice eligibility assessments based on thorough knowledge of Medicare Hospice Regulations.
- Educate patients, families, and facility staff about hospice services and patient-specific care plans.
- Act as a primary point of contact to ensure coordination and continuity of care.
- Utilize strong communication and time management skills to effectively manage multiple cases and priorities.
- Participate in interdisciplinary team meetings to review patient plans and compliance with hospice criteria.
- Provide ongoing support and education on the hospice process to healthcare providers within the community.
Qualifications:
- Must possess an active RN license.
- Proven hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.
- Exceptional interpersonal, communication, and presentation skills.
- Robust organizational and time management skills are essential.
- Must be adaptable, patient-focused, and capable of handling challenging situations with tact and empathy.
Job Type: Full-time
Pay: $75,000.00 - $85,000.00 per year
