Advanced Home Health and Hospice is seeking a compassionate, skilled, and dependable Hospice RN Case Manager to join our hospice team. This is an opportunity for a nurse who wants to do meaningful work, build relationships with patients and families, and provide comfort, dignity, and support during one of the most important stages of care.

About the Role

As a Hospice RN Case Manager, you will coordinate and manage care for patients in a hospice setting while working closely with an interdisciplinary team. You will provide skilled nursing services, support families, manage symptoms, and help ensure each patient receives personalized, respectful, and comfort-focused care.

Responsibilities

• Conduct comprehensive patient assessments to identify physical, emotional, psychosocial, and care-related needs.
• Develop, implement, and update individualized care plans based on patient needs, physician orders, and hospice goals of care.
• Provide skilled nursing care, including pain and symptom management, medication administration, patient education, and ongoing clinical monitoring.
• Support patients and families with education, guidance, and emotional reassurance throughout the hospice journey.
• Monitor each patient’s physical, emotional, and spiritual well-being while helping preserve comfort and dignity.
• Communicate regularly with patients, families, physicians, hospice aides, and members of the interdisciplinary team.
• Coordinate and supervise care provided by hospice aides and other team members involved in the patient’s care.
• Collaborate with the interdisciplinary team to develop care strategies that align with each patient’s wishes and goals.
• Participate in team meetings to review care plans, discuss patient progress, and address barriers to care.
• Ensure timely and accurate documentation in accordance with hospice regulations, agency policies, and care standards.
• Provide bereavement support and connect families with appropriate resources as needed.
• Maintain compliance with regulatory guidelines, documentation requirements, and company policies.
• Perform other duties as assigned by the Hospice Director or designated supervisor.

Qualifications

• Current Registered Nurse license in the state of practice.
• Minimum of two years of RN experience in hospice or a related healthcare setting.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver’s license and reliable transportation for home visits.
• Current CPR certification.
• Knowledge of hospice regulations, documentation standards, and care guidelines.
• Commitment to compassionate, patient-centered end-of-life care.
• Experience with electronic medical record systems preferred.

Required Skills

• Ability to lift, transfer, and assist patients as needed.
• Ability to carry equipment and supplies weighing up to 50 pounds.
• Ability to stand, walk, bend, squat, and move throughout the day during patient visits.
• Fine motor skills for documentation, medication administration, and treatment-related tasks.
• Ability to respond quickly and effectively in urgent or emergency situations.
• Ability to maintain regular attendance and punctuality according to the assigned schedule.
• Ability to remain calm, professional, and effective in high-stress situations.
• Ability to use core, abdominal, and lower back muscles repeatedly or continuously during patient care activities.

Pay range and compensation package

Full-Time | Monday–Friday | Day Shift | 8-Hour Shifts
