Advanced Home Health and Hospice is seeking a compassionate, skilled, and dependable Hospice RN Case Manager to join our hospice team. About the Role
As a Hospice RN Case Manager, you will coordinate and manage care for patients in a hospice setting while working closely with an interdisciplinary team. You will provide skilled nursing services, support families, manage symptoms, and help ensure each patient receives personalized, respectful, and comfort-focused care.
Responsibilities
- Conduct comprehensive patient assessments to identify physical, emotional, psychosocial, and care-related needs.
- Develop, implement, and update individualized care plans based on patient needs, physician orders, and hospice goals of care.
- Provide skilled nursing care, including pain and symptom management, medication administration, patient education, and ongoing clinical monitoring.
- Support patients and families with education, guidance, and emotional reassurance throughout the hospice journey.
- Monitor each patient’s physical, emotional, and spiritual well-being while helping preserve comfort and dignity.
- Communicate regularly with patients, families, physicians, hospice aides, and members of the interdisciplinary team.
- Coordinate and supervise care provided by hospice aides and other team members involved in the patient’s care.
- Collaborate with the interdisciplinary team to develop care strategies that align with each patient’s wishes and goals.
- Participate in team meetings to review care plans, discuss patient progress, and address barriers to care.
- Ensure timely and accurate documentation in accordance with hospice regulations, agency policies, and care standards.
- Provide bereavement support and connect families with appropriate resources as needed.
- Maintain compliance with regulatory guidelines, documentation requirements, and company policies.
- Perform other duties as assigned by the Hospice Director or designated supervisor.
Qualifications
- Current Registered Nurse license in the state of practice.
- Minimum of two years of RN experience in hospice or a related healthcare setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver’s license and reliable transportation for home visits.
- Current CPR certification.
- Knowledge of hospice regulations, documentation standards, and care guidelines.
- Commitment to compassionate, patient-centered end-of-life care.
- Experience with electronic medical record systems preferred.
Required Skills
- Ability to lift, transfer, and assist patients as needed.
- Ability to carry equipment and supplies weighing up to 50 pounds.
- Ability to stand, walk, bend, squat, and move throughout the day during patient visits.
- Fine motor skills for documentation, medication administration, and treatment-related tasks.
- Ability to respond quickly and effectively in urgent or emergency situations.
- Ability to maintain regular attendance and punctuality according to the assigned schedule.
- Ability to remain calm, professional, and effective in high-stress situations.
- Ability to use core, abdominal, and lower back muscles repeatedly or continuously during patient care activities.
