As a per-diem Wellness Nurse serving independent living communities in Marshfield and Duxbury, you'll build real relationships with residents, deliver health education, and provide the kind of proactive, personalized care that makes a lasting difference. This is an autonomous, community-based role, ideal for a nurse who values independence, meaningful work, and flexible scheduling.
What You'll Do
- Conduct wellness assessments and health screenings, including vital signs monitoring
- Deliver health education and wellness coaching on topics like healthy aging, fall prevention, chronic disease management, medication safety, and nutrition
- Build trusting relationships with residents and encourage engagement in wellness programs
- Identify residents who may need additional support and coordinate appropriate referrals
- Collaborate with Resident Service Coordinators, property management teams, families, and healthcare providers
- Maintain accurate, timely electronic documentation
What We're Looking For
- Current Massachusetts LPN or RN license in good standing
- Background in community health, home care, assisted living, VNA, or primary care preferred
- Current CPR certification
- Strong interpersonal and communication skills
- Reliable transportation and comfort traveling between assigned communities
- Self-directed and confident working independently
- Bilingual candidates are warmly encouraged to apply
