Key Responsibilities:
- Evaluate residents and develop individualized treatment plans
- Deliver evidence-based physical therapy services
- Document progress and adjust treatment plans as needed
- Collaborate with physicians, nursing staff, and other rehab team members
- Educate residents and families on therapy goals and home exercises
- Current and valid Physical Therapist (PT) license in CT
- Graduate of an accredited Physical Therapy program
- Previous experience in SNF or geriatric setting preferred (new grads welcome!)
- Strong communication and interpersonal skills
