Adult Day Health Care of Madera is seeking two (2) Medical Social Worker for social work services for participants. Duties and Responsibilities:
- Provide all social work services for participants.
- Meets with family members or referring entities and determines appropriate program.
- Completes social service assessments on all participants.
- Ensures person centered care planning, includes participant, family members and/or caregivers in the development of care plan.
- Provides discharge planning for all participants.
- Provides participant and family counseling as needed.
Qualifications:
- Master's degree in social work from an accredited school of social work or a master's degree in psychology, gerontology, or counseling from an accredited school
- One (1) year of experience providing social services in one or more of the fields of aging, health, or long-term care services or be licensed by the California Board of Behavioral Sciences.
- Bilingual is highly preferred.
Compensation is based on experience.
Hours of Operation: Monday through Friday, 8:00 am-4:30 pm.
