Job Overview:
We are currently looking for a dedicated and compassionate Registered Nurse (RN) to join our interdisciplinary team providing care to frail and elderly adults. The RN will play a key role in delivering quality nursing care, performing participant assessments, coordinating individualized care plans, managing medications, providing health education, and maintaining compliance with healthcare regulations and safety standards.
Qualifications
- Valid Registered Nurse license
- Experience in public health or geriatric nursing is highly preferred.
- Strong clinical, communication, organizational, and documentation skills.
- Ability to work collaboratively within a multidisciplinary healthcare environment.
Requirements
- Bilingual skills in English and Chinese (Mandarin and/or Cantonese)
- Must provide physician clearance completed within the last 12 months confirming ability to perform job responsibilities. Proof of a negative TB test completed within the last 12 months is required.
- Current CPR and First Aid certification
Responsibilities
Duties include, but are not limited to:
- Perform initial and ongoing nursing assessments to identify participant needs and develop individualized care plans with both short- and long-term goals.
- Administer and monitor prescribed medications while ensuring proper documentation and regulatory compliance.
- Observe participants’ health conditions and communicate significant changes to the care team and physicians.
- Coordinate with primary care providers to ensure continuity and quality of care.
- Evaluate personal care requirements and oversee assistance with activities of daily living (ADLs).
- Maintain accurate and timely documentation of nursing services, progress notes, and participant care records.
- Deliver supportive, preventive, and restorative nursing care according to individualized plans of care.
- Develop and monitor continence care and training programs.
- Educate staff regarding disease management, infection prevention, participant safety, and proper care procedures.
- Oversee medication storage, handling, and compliance with applicable regulations.
- Teach participants safe self-administration of medications when appropriate.
- Work collaboratively with rehabilitation staff to support therapy maintenance programs.
- Provide education and counseling to participants and their families regarding health and wellness.
- Assist participants in obtaining medical care and accessing community health resources.
- Support and supervise nursing students as assigned.
- Ensure adherence to all organizational policies, infection control guidelines, fire safety procedures, licensing requirements, and emergency preparedness standards.
- Participate in required orientation sessions and ongoing staff training programs.
