Position Overview
ABC Therapy Center is seeking a compassionate and experienced Social Worker to join our Community-Based Adult Services (CBAS) / Adult Day Health Care (ADHC) team. The Social Worker plays a vital role in assessing participants' psychosocial needs, coordinating community resources, and supporting participants and their families through the care continuum.
Key Responsibilities
- Conduct initial and ongoing psychosocial assessments for CBAS participants
- Develop, implement, and update individualized care plans (Care Plans) in collaboration with the interdisciplinary team
- Provide counseling and supportive services to participants and their families
- Identify and connect participants with community resources, including housing, benefits, and caregiver support programs
- Assist with Medi-Cal eligibility, applications, and benefit coordination
- Facilitate care transitions, including hospital discharge planning and referrals
- Maintain accurate and timely case notes and documentation in compliance with Title 22 and CDPH requirements
- Participate in interdisciplinary team (IDT) meetings and care conferences
- Advocate for participants' rights, dignity, and quality of life
Qualifications
- Bachelor's degree in Social Work (BSW) required; Master's degree (MSW) preferred
- Current California licensure (LCSW, ACSW, or LMSW) preferred
- Minimum 1 year of experience in geriatric, community health, or social services setting preferred
- Familiarity with Medi-Cal, Medicare, and community-based services for older adults
- Knowledge of Title 22 and CBAS/ADHC regulations a plus
- Strong interpersonal, communication, and documentation skills
- Cantonese proficiency required — ability to communicate effectively with Cantonese-speaking participants and families
Pay: $22.00 - $28.00 per hour
