Position Overview
We are seeking a compassionate and dedicated Registered Nurse to join our Community-Based Adult Services (CBAS) / Adult Day Health Care (ADHC) team. The RN will provide skilled nursing care to our adult and elderly participants, ensuring high-quality, person-centered services in compliance with state regulations.
Key Responsibilities
- Conduct health assessments, monitor vital signs, and maintain accurate nursing documentation
- Administer medications and carry out physician orders in accordance with care plans
- Develop, implement, and update individualized care plans (Care Plans) in collaboration with the interdisciplinary team
- Identify and respond to changes in participants' health conditions; escalate as appropriate
- Ensure compliance with CDPH, and Medi-Cal regulations
- Provide health education and maintain ongoing communication with participants and their families
- Coordinate with case managers, social workers, and therapists to support holistic care
Qualifications
- Valid California RN License (active and in good standing)
- Current CPR/BLS certification
- Minimum 1 year of nursing experience; geriatric or community health experience preferred
- Strong knowledge of chronic disease management and elderly care
- Ability to maintain accurate medical records and documentation
- Cantonese proficiency required — ability to communicate effectively with Cantonese-speaking participants and families
- Spanish proficiency a plus
