- Responsibilities
- Develop and implement individualized care plans tailored to each participant’s needs, including memory care, dementia support, and acute pain management strategies.
- Coordinate multidisciplinary team efforts involving social workers, nurses, assistants, and external healthcare providers to deliver holistic care.
- Oversee daily center operations ensuring compliance with HIPAA regulations, safety protocols, and documentation standards using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic or Athenahealth.
- Conduct comprehensive patient assessments including vital signs, physical examinations, medication administration, and diagnostic evaluations like IV infusion or catheter care.
- Manage intake processes, discharge planning, and case management activities to optimize participant outcomes and resource utilization.
- Provide staff supervision and training in areas such as medical documentation, infection control, sterile processing techniques, and emergency response procedures including basic life support (BLS).
- Foster a supportive environment for individuals with complex conditions such as Alzheimer’s disease or other memory impairments through specialized programming and staff education.
Requirements
- Valid licensure as a Registered Nurse (RN), Licensed Vocational Nurse (LVN), or Social Worker (SW) with current state certification.
- Proven experience in adult or senior care settings such as assisted living facilities, nursing homes, hospitals—including ICU or trauma centers—and home care environments.
- Familiarity with medical terminology, ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or spinal taps, and proficiency with EMR/EHR systems like Cerner or eClinicalWorks.
- Critical care experience preferred; knowledge of acute pain management, infectious disease care, hospice services, and emergency medicine is highly valued.
- Strong background in working with diverse populations including those with developmental disabilities or behavioral health needs; experience in dementia or Alzheimer’s care is advantageous.
- Ability to perform patient assessments involving physiology knowledge, airway management techniques such as ventilator use or tube feeding procedures.
