Job Overview
We are seeking a compassionate, dependable, and experienced Certified Nursing Assistant (CNA) to provide one-on-one support to an adult male with significant physical and medical needs in his home and community. The individual is completely dependent on staff for most activities of daily living and requires attentive, person-centered care to ensure his health, safety, dignity, and quality of life.
The ideal candidate will have experience caring for individuals with complex medical needs, including mobility impairments, seizure disorders, G-tube care, and safe transfers. The schedule is as follows: Monday 4PM-11PM, Friday 3PM-11PM, rotating weekends (Saturday & Sunday) 8AM-11PM.
Responsibilities- Provide complete assistance with activities of daily living (ADLs), including:
Mobility & Transfers:
- Perform safe transfers using a Hoyer lift according to company policy and training.
- Assist with wheelchair positioning and secure all safety devices.
- Provide two-person assistance during transfers when required.
- Ensure proper body mechanics and follow all lifting protocols.
- Assist with range-of-motion exercises as directed.
Medical & Health Support:
- Monitor for seizure activity and respond according to the individual's seizure protocol.
- Observe for signs of aspiration, respiratory distress, illness, skin breakdown, or changes in condition.
- Administer medications as delegated and trained under company policy.
- Assist with medications administered through a G-tube following physician orders and company procedures.
- Monitor hydration and nutritional intake as directed.
- Document all health concerns immediately.
Feeding & Nutrition:
- Prepare meals according to dietary recommendations.
- Assist with feeding as needed.
- Follow aspiration precautions and ensure safe eating practices.
- Maintain proper positioning during meals and medication administration.
- Bathing
- Dressing
- Grooming
- Oral hygiene
- Toileting and incontinence care
- Skin care and positioning
- Assist with all hygiene needs while maintaining dignity and privacy.
- Reposition the individual frequently to promote skin integrity and prevent pressure injuries.
Documentation:
- Complete all documentation accurately and before the end of each shift using Therap.
- Record:
- T-Logs
- Goal progress
- Health observations
- Medication documentation (if applicable)
- Incident reports when necessary
Qualifications
- Current Pennsylvania CNA Certification
- Experience working with individuals with developmental disabilities is highly preferred.
- Minimum of 18 years of age, required
- A valid driver’s license is required
- Current car insurance and registration, required
- Successfully pass a background check, required
