This position is focused exclusively on primary care. What You'll Do
- Provide comprehensive outpatient primary care for patients across the lifespan, including infants, children, adolescents, adults, and older adults
- Diagnose and treat acute illnesses and injuries
- Manage chronic medical conditions using evidence-based practices
- Perform comprehensive patient evaluations, histories, and physical examinations
- Deliver preventive care, wellness visits, immunizations, and routine health maintenance
- Perform Medicare Annual Wellness Visits and age-appropriate screenings
- Develop individualized treatment plans and coordinate care with specialists and community resources
- Educate patients and families regarding disease prevention, healthy lifestyle choices, and chronic disease management
- Collaborate with the medical director and interdisciplinary care team to deliver coordinated, patient-centered care
- Maintain accurate, timely, and compliant documentation within EHR

Qualifications
Required
- Board-certified family nurse practitioner or physician assistant
- Active Michigan license (or ability to obtain)
- DEA registration (or ability to obtain)
- Ability to provide comprehensive primary care across the lifespan
- Two or more years of outpatient primary care experience
- Experience practicing independently in a primary care setting
- Strong clinical judgment and patient-centered approach
- Certified DOT medical examiner
- Excellent communication and relationship-building skills
- Interest in rural or community-based healthcare
