Safety

Photocopying and maintaining inventory of forms, packets, supplies, including safety supplies such as PPE. Ensuring location safety compliance by acting as local Safety Officer and as a member of the Adjoin Safety Committee including:. Track and facilitate required safety training and certification such as First Aid/CPR. Complete safety orientation training for new hires.

Community

Facilitating the client’s inclusion in positive community experiences. Maintaining positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community. Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.

Management

Managing and assisting with documentation by scanning, uploading, organizing and filing as needed. Maintaining and manage petty cash in addition to tracking purchases and receipts. Manage and oversee all safety matters in the office, including fire extinguishers, first aid kits & safety supplies. Participate in and apply information from quarterly organization-wide Safety Committee meetings. Coordinate, participate in and follow up on both internal and external office inspections.

Maintain and update safety documents and required postings. Overseeing the maintenance of all office equipment, assuring office space is well maintained, orderly and a professional environment, maintaining inventory of office supplies, equipment and furniture.

Implementing individual training and support programs under the direction of the case responsible person. Addressing challenging behaviors proactively and respectfully, using methods outlined in plans and training.

Plan, conduct, and ensure staff attendance at month safety meetings. Plan, execute and track safety drills.

Participating in the evaluation of the client through completion of task analysis and other requested assessment devices.

Assist

Promoting independence in daily activities while being mindful of client’s intellectual and physical disabilities and assisting with meals, cooking, household maintenance, grooming and dressing, as assigned.

Overseeing client activities and environment throughout shift to ensure the fulfillment of health and safety needs. Participating in Quality Assurance activities per company schedule as assigned.

Assisting with Pre-Hire and onboarding process for new hires as needed and as directed by management.

Requirements

Experience

18 years of age or older. High School Diploma or Equivalent. Aptitude for confidentiality, organizational skills and good interpersonal skills. Certification in an approved First-Aid and CPR class; or successful completion within 45 days of the date of hire. Insured, registered vehicle in safe working condition, and proof of current CA driver’s license.

Mobile

Adjoin’s client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.

Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.

Schedule

A Direct Support Facilitator works as scheduled by the Regional Manager and/or his/her designee. His/her schedule may be either part or full time, depending on the needs of the clients and Adjoin. Overtime may be required as dictated by operational needs, however, no holiday or overtime may be worked without the prior approval of the Direct Support Professional's supervisor.

Compliance

Successful completion of pre-employment drug screen and background check. Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test. Acceptable DMV record.
