A skilled nursing facility located in Yonkers is seeking an experienced Per Diem Nurse Practitioner to join our team.The Nurse Practitioner (NP) will be responsible for working in collaboration with the medical director and attending physicians in the medical management of residents admitted to the Skilled Nursing Facility/Sub-Acute Rehab unit. The Nurse Practitioner will assess, diagnose and treat the residents, according to their scope of practice and NYS Law.The Nurse Practitioner will take/review patients medical histories, conduct physical examinations, write admission and follow up notes in the EMR, order and interpret labs and diagnostic studies, diagnose and treat medical conditions, make referrals when needed and prescribe medications including controlled medications.Minimum RequirementsMaintains current nursing and advanced practice license in compliance with State lawMaintains current DEA licensureAbility to develop and maintain collaborative relationshipsExperience in working with older adults with complex medical conditionsAbility to assess subtle changes in health conditions of the older adultWorking knowledge of State and Federal Long Term Care regulationsExcellent administrative, organizational and verbal skillsEffective communication skills with the geriatric populationAbility to work independentlyDetail orientedDependable and reliableProficiency in EMR systems (Sigma/MatrixCare a Plus)Preferred QualificationsA clinical background in adult, family or geriatric practicePast experience working in a nursing home or with older adults in other settingsExperience in working in a post-acute setting
Salary : $144,320 - $174,820
