Assisted Living Nurse Consultant

Adept Nursing and Rehab of South Sioux City is Now Hiring a Part-time ALF Nurse Consultant

Key Responsibilities

• Provide ongoing clinical consultation and nursing oversight for residents.

• Review resident health conditions, care needs, and changes in condition.

• Assist with development, implementation, and review of individualized service and care plans.

• Monitor medication management practices and provide guidance to staff as appropriate.

• Evaluate incidents, falls, changes in condition, and other clinical concerns.

• Communicate with physicians, pharmacies, home health agencies, hospice providers, families, and other healthcare professionals.

• Provide training, education, and clinical support to caregivers and community staff.

• Promote infection prevention, resident safety, and quality improvement practices.

• Review nursing documentation and ensure accurate and timely clinical records.

• Assist with admissions, hospital returns, and transitions of care as needed.

• Help ensure the community follows applicable state regulations, policies, and standards of care.

• Respond to clinical questions and provide recommendations to the Executive Director or Administrator.

• Maintain confidentiality and protect residents' privacy and dignity at all times.

Qualifications

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing

• Previous experience in assisted living, senior care, long-term care, home health, geriatrics, or a related healthcare setting preferred.

• Strong assessment, communication, organization, and critical-thinking skills.

• Knowledge of medication management, care planning, dementia care, fall prevention, and chronic disease management.
