As a Health & Wellness Lead, you’ll help create a warm, engaging environment where residents feel supported and valued. You’ll provide direct nursing care while guiding and supporting the care team to deliver exceptional service each day. Working closely with our Health & Wellness Director, you’ll ensure that residents receive individualized care that promotes wellness, safety, and quality of life.
What You’ll Do:
- Lead and support the daytoday nursing and care activities within the community.
- Provide direct resident care, including ADLs (Activities of Daily Living) and medication supervision, following physician orders and care plans.
- Partner with the Health & Wellness Director to coordinate staffing, develop care plans, and evaluate resident care.
- Accurately document services, observations, and progress notes using the appropriate forms or electronic system.
- Monitor and report changes in residents’ health conditions and communicate effectively with the care team.
- Assist residents with maintaining good personal hygiene and overall wellness.
- Promote a safe, clean, and organized environment for residents and team members.
- Record and track vital signs (temperature, blood pressure, pulse, respiration) monthly or as directed.
- Build meaningful relationships with residents, families, and fellow team members through clear, compassionate communication.
What We’re Looking For:
- Active Licensed Practical Nurse (LPN) in good standing (State of Florida).
- 12 years of experience as an LPN in an Assisted Living or Senior Living setting.
- Strong leadership and teamwork abilities.
- A caring and patientcentered approach to care.
- Excellent communication and problemsolving skills.
