- Graduation from a professional nursing program accredited by a recognized nursing body (e.g., ACEN or CCNE).
- Active and unrestricted Registered Nurse (RN) license in any State or U.S. territory
- Minimum of two (2) years of direct patient care experience within the last three (3) years.
- Current BLS certification.
- Clinically review consults/requests for completeness and appropriateness/utilization review.
- Dispositioning care in the community per utilization review and guidelines.
- Clinical review of incoming medical records documenting communication with ordering providers and PCP.
- Communicate with veterans, community providers, internal staff, and administrative teams to facilitate and process care authorizations.
- Explain care options to veterans and assist in making informed decisions regarding community-based care.
- Document communication and care coordination efforts in the electronic health record (EHR).
- Any other duties assigned.
