As an LPN at Adelante you’ll be at the heart of our mission, working directly with individuals on the DD Waiver. Every day brings the opportunity to use your nursing skills while building meaningful connections. Here’s what your work will look like:
What You’ll Do:

Create personalized healthcare plans that truly support each individual’s needs

Train and support staff so they feel confident providing care

Conduct nurse visits and attend medical appointments with clients

Participate in ISP & IDT meetings, ensuring each person’s voice is heard
-
Maintain accurate medication and treatment records to keep care safe and consistent
What We’re Looking For:

Licensed Practical Nurse (LPN) certified in New Mexico
-
At least 1 year of experience working with individuals with disabilities (experience with severe/profound disabilities is a big plus!)

Bonus points if you have experience with feeding tubes (jejunostomy/gastrostomy)
What You’ll Need:

Reliable transportation, valid NM driver’s license, and proof of insurance

Ability to lift up to 50 lbs and be on the move visiting clients and sites

Flexibility to work occasional non-scheduled hours when needed

Completion of background checks, screenings, and required trainings
