What are the responsibilities and job description for the LPN position at Addictions Care Center of Albany, Inc?
Responsibilities:
- Participates in screening of clients' medical needs, gathering necessary admission information/data, and observation of physical, mental, and emotional status of residential clients.
- Works cooperatively with managerial and medical staff to meet the treatment needs of each client
- Provides group counseling sessions to clients regarding health-related issues and concerns
- Observe that all medications are properly taken by clients
- Document medications taken in MAR and 10e11.
- Take vitals of clients as ordered by doctor.
- Gather and submit all urine drug screens to the labs
- Document in client 10e11 charts all lab results in a timely fashion
- Monitor clients for medical needs and report findings to medical provider
- Ensures confidentiality and safety/security of all residential clients.
- Logs and verbally receives/delivers accurate information at change of shifts.
- Prepares necessary reports/records as required by program procedure.
- Participates in weekly case conferences as needed and/or directed by supervisor
- Assumes responsibility reporting incidents involving client medical occurrences.
- Make calls to local ambulance service, fire department, police, or CDPC, as needed.
- Attends required staff meetings and training.
- Performs other duties as assigned and works in other residential buildings as needed.
Education Requirements: Accredited L.P.N. or R.N. program.
License Requirements: Candidate must have valid NYS LPN or R.N. license, and NYS Driver's License. Must be CPR and First Aid certified.
Experience Requirements: Two (2) years nursing experience preferred, preferably in a NYS OASAS residential facility.
Shift is Monday - Friday 7am-3pm
