The Physical Therapist will be responsible for delivering physical therapy services to clients in their home, adhering to the Plan of Care and company policies. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Key Responsibilities:
- Develops and implements physical therapy plans tailored to clients' needs
- Provides physical therapy treatments, procedures, evaluations, and diagnostic tests for clients under physician direction
- Collaborates with other healthcare professionals to ensure comprehensive care
Qualifications:
- Graduate of an accredited Physical Therapy program; currently licensed as a Physical Therapist in Minnesota
- At least two years of experience in a healthcare setting or equivalent experience
- Strong leadership and team collaboration skills
