Role Description
This is a full-time on-site role for a Home Care Physical Therapist located in Toms River, NJ. The Physical Therapist will assess patients’ physical conditions, develop individualized treatment plans, and provide therapeutic interventions to address mobility and functionality goals. The role also involves educating patients and caregivers, monitoring progress, and maintaining accurate records to ensure exceptional patient care.
Qualifications
- Experience in Physical Therapy, Rehabilitation, and Home Care settings
- Strong skills in Medicine and Patient Care with a focus on delivering personalized treatment
- Licensed Physical Therapist with certification to practice in the state of New Jersey
- Proficiency in assessing mobility challenges and designing tailored recovery programs
- Excellent interpersonal and communication skills to effectively interact with patients, caregivers, and team members
- Ability to work independently and manage a caseload efficiently
- Prior experience in outpatient therapy or home care is an advantage
