This role ensures timely clinical decision-making, appropriate escalation, and continuity of care for home health patients requiring after-hours intervention. The RN serves as the first-line clinical resource for patients, caregivers, field clinicians, and facility partners, utilizing strong assessment skills and sound clinical judgment. This position is 100% remote telephonic triage only with no in-home visit requirements.

Key Responsibilities
Clinical Triage & Patient Support
- Respond promptly to after-hours patient calls and urgent clinical needs.
- Conduct telephonic assessments to determine urgency and needed interventions.
- Provide clinical guidance for symptoms, medications, wound issues, pain, and condition changes.
- Document all triage calls thoroughly in the EMR.
- Determine need for in-person visits and coordinate with on-call RN/CM.
- Escalate emergencies to 911 or urgent care resources when needed.
- Notify Clinical Manager or on-call leadership for escalations.
- Communicate recommendations and follow-up needs to branch leaders and field teams.
- Collaborate with weekend/evening leadership for continuity of care.
- Review orders, labs, and concerns arising after hours and act per policy.
- Ensure timely physician communication for urgent situations.
- Complete documentation within required timeframes.
- Follow HIPAA and home health regulatory standards.
- Provide calm, compassionate, patient-centered communication.
- Demonstrate strong problem-solving and clinical leadership.
- Represent the agency professionally.
Required
- Active RN license.
- 1–2 years recent clinical experience (home health, ER, urgent care, med-surg, or ICU preferred).
- Strong triage and telephonic assessment skills.
- EMR proficiency (HCHB preferred).
- Reliable internet and ability to work independently.
- Prior home health on-call/triage experience.
- Knowledge of Medicare and home health regulations.
