The mission of this role
Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
Objectives
- Carry a full, high-quality caseload: ramp to ~28 productivity points/week at target visit quality.
- Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
- Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
- Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
- Complete in-home OT evaluations and deliver skilled OT visits.
- Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
- Recommend and train on adaptive equipment and home modifications as needed.
- Coordinate with nursing and the broader care team when condition or needs change.
- Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
- Dedicated clinician: home health experience strongly preferred

- Active Michigan OT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfort escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Valid driver’s license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory.
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
- Prior home health experience
- Homecare Homebase (HCHB) experience (or similar home health EMR)
- Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
- Strong organization and time management
- Full-time target: ~28 productivity points/week; clinicians earn more for exceeding it.
- Guaranteed floor: ~24 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor.
- Mileage reimbursement at the IRS rate (~$0.67/mi)
- $1k sign-on bonus paid after first 90 days
- $1k bonus for referrals (we’re growing)!
