An employee in this position shall assist the Community Habilitation Coordinator in overseeing the development and growth of the Community Habilitation programs with an emphasis on quality and person-centered practices. An employee in this position is responsible for overseeing the compliance to OPWDD regulations and ADAPT of the Hudson Valley guidelines assigned to the Community Habilitation program. The incumbent will also provide direct support and serves as a role model, advocate and teacher for the people we support.

Position Responsibilities:

  • Works closely with the Community Habilitation Coordinator to promote an environment that encourages and promotes independence and individualization in all aspects of the individuals’ we support lives.
  • Maintain a reasonable case load and supervises Community Habilitation Direct Support Professionals. Responsible for the well-being of the individuals in a community setting. Exhibits sensitivity to and respects the rights of the individuals served, including their emotional and physical needs.
  • Ensures a full range of recreational activities. Facilitates community integration and inclusion by locating activities of interest to individuals supported while exploring alternate avenues of interest.
  • Communicates with Residential Managers, families, advocates, Care Managers, and all relevant team members to ensure the adequate delivery of Community Habilitation services.
  • Participates in therapeutic team meetings and the development of habilitation plans, behavior plans and safeguards.
  • Maintain ongoing communication with the individual, therapeutic team, families and supervisor.
  • Provide training, supervision, assistance and monitoring in areas related to the acquisition and retention of daily living skills and self-care skills.
  • Responsible for the implementation and monitoring of the Community Habilitation plan.
  • Monitor the data collection and ensure the billing is submitted on time.
  • Provide supervision to the Direct Support Professionals.
  • Monitor and ensure all staff training/certifications are current at all times.
  • Assist with the development of the staff schedule and ensure staff are held accountable for performance.
  • Maintain current CPR, FA, SCIP certifications.
  • Perform other duties as assigned or directed.
Requirements:
  • Associates degree and 2 years’ experience or High School/GED with at least five years’ experience with the population or human services field.
  • Some management or supervisory experience preferred.
  • Have the ability to speak, read, and write English fluently which allows full communication with people supported; ADAPT HV staff; and, 3rd parties.
  • Have the ability to relate appropriately to individuals with developmental disabilities.
  • Computer proficiency, particularly with Microsoft Office applications and the ability to learn to use Medisked and other database systems.
  • Ability to work independently and as part of a team.
  • Ability to use sound judgment while following ADAPT HV policies, practices and procedures.
  • Ability to operate office equipment (including computers).
  • Possess a valid driver license with a driving record acceptable to ADAPT HV’s insurance carrier and possess the ability to travel to all ADAPT HV sites throughout the areas we serve.
  • Successfully meet all screening requirements (both prior to hire and during employment) as required by applicable regulation.

Nonessential Skills and Experience:

Prior work experience using Medisked (or similar databases).

Salary Range: $19.38 - $21.00 per hour
