placement, coaching, and long-term employment success through person-centered planning,
community partnerships, and ongoing mentoring. The Specialist plays a critical role in promoting
independence, inclusion, and career growth while supporting ADAPT Community Network’s
mission of empowerment and opportunity.

The duties and responsibilities of the Supported Employment Specialist will include, but are not

- Develops individualized employment plans based on each person’s skills, interests,
- Assists individuals with resume development, interview preparation, job readiness
- Supports individuals in identifying and pursuing competitive employment opportunities
- Conducts employer outreach and job development activities to build relationships with
- Coordinates job placements and facilitates successful onboarding and workplace
- Provides on-the-job coaching, mentoring, and skill reinforcement to support long-term
- Conducts regular follow-ups with individuals and employers to monitor progress,
- Collaborates with families, care managers, vocational providers, and internal team
- Maintains accurate, timely, and detailed documentation including service notes, progress
requirements.
- Ensures services are delivered in compliance with OPWDD, HCBS Waiver, and agency
- Stays informed of employment laws, labor regulations, and best practices related to disability employment and workforce development.
- Promotes self-advocacy, independence, and confidence in all service interactions.
- Participates in required meetings, trainings, and professional development activities.
- Maintains strict confidentiality in accordance with HIPAA and agency standards.
- Performs other duties as requested.

- High School Diploma is required; College Degree preferred.
- Proven experience working with people with developmental disabilities, preferably in
- Background in social work, addiction counseling, teaching, or related fields is preferred.
- Knowledge of employment & labor law as it pertains to individuals with disabilities.
- Strong understanding of workforce development strategies and inclusive hiring practices.
- Excellent interviewing, mentoring, and training skills to support client growth.
- Ability to read, write, speak and understand English.
- Good oral and written communication skills.
- Good interpersonal skills necessary to interact effectively with coworkers, employees, the
