Summary

Under supervision of the Program Director, the Case Manager is responsible for developing, implementing, and monitoring Life Plans for the people we support in one of our residential settings. This role ensures that services align with regulatory requirements, agency standards, and the personal goals of each individual. The Case Manager provides advocacy, coordination of services, and family engagement, while promoting dignity, independence, and quality of life for the people we support.

Essential Duties And Responsibilities

The duties and responsibilities of the Case Manager will include, but are not limited to the following:

  • Provides social services to each person we support working toward maximizing their social functioning, enhancing coping capacity of their family, asserting and safeguarding their human and civil rights, and fostering their human dignity and personal worth.
  • Develops, monitors, and updates Individualized Service Plans in accordance with regulatory and agency policies.
  • Participates in and chairs interdisciplinary team meetings; documents, interprets, and follows up on all recommendations.
  • Collaborates with residential staff, clinical staff, and service providers to ensure coordinated support.
  • Identifies individual aptitudes, interests, and preferences and develops goals and activities that reflect person-centered planning.
  • Documents functional assessments and reviews plans of care as needed.
  • Provides supervision, training, and guidance to Residence Program Specialists on Life Plan implementation, data collection, and best practices.
  • Maintains regular and meaningful contact with families, guardians, and community stakeholders.
  • Advocates for the rights of the people we support and ensures benefits and entitlements are applied for in a timely manner.
  • Coordinates admissions, including referral materials, family contacts, and arrangements for day programming.
  • Serves as liaison to day programs, clinics, service coordination, and other external partners.
  • Provides support in activities of daily living and community outings as needed.
  • Ensures adaptive equipment is available, maintained, and used appropriately.
  • Maintains accurate, timely documentation, including monthly case notes, progress updates, and family contacts.
  • Assists with discharge planning and systematic follow-up to ensure smooth transitions to community-based services.
  • Handles sensitive and confidential information with the highest level of discretion.
  • Keeps abreast of changes in the field and applies updated knowledge to practice.
  • Performs other related duties as assigned.

Qualifications

To perform this job successfully, an incumbent must be able to perform each essential duty satisfactorily.   • Bachelor’s Degree in Human Services or related field required.
  • Minimum of one (1) year of experience working with individuals with developmental disabilities required.
  • Ability to read, write, speak, and understand English.
  • Strong interpersonal and communication skills with the ability to interact effectively with people supported, families, staff, and outside agencies.
  • Excellent organizational and time management skills with the ability to handle multiple priorities.
  • Proficiency in Microsoft Office and electronic record systems.
  • Ability to maintain confidentiality and exercise discretion in handling sensitive information.
  • Ability to work independently, as part of a team, and with flexibility in scheduling.
  • Ability to work flexible hours.

COMPENSATION: $28.34 - $29.43/hour (after probation) Industry-Leading Benefits for all full-time employees.
