Duties
- Conduct thorough patient evaluations to determine functional limitations and develop tailored treatment plans aligned with clinical standards.
- Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics interventions, neurology rehabilitation, and geriatrics-focused therapies.
- Manage electronic health records (EHR) and electronic medical records (EMR) systems to document patient progress accurately and efficiently.
- Coordinate discharge planning and collaborate with multidisciplinary teams to ensure seamless patient care transitions across settings.
- Provide patient education on injury prevention, treatment exercises, and home care strategies to maximize recovery outcomes.
- Assist in outpatient rehabilitation programs by guiding patients through therapy sessions aimed at improving mobility and strength.
- Monitor patient progress regularly, adjusting treatment plans as necessary to meet evolving needs and goals.
Requirements
- Valid licensure as a Physical Therapist in accordance with state or national licensing boards.
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Experience working with diverse populations including geriatrics, pediatrics, neurology patients, and those requiring orthopedics or inpatient care.
- Familiarity with outpatient rehabilitation center operations and inpatient hospital workflows.
- Ability to communicate effectively with patients and collaborate seamlessly with healthcare professionals across disciplines.
- Proficiency in managing electronic health records (EHR) systems such as EMR platforms for accurate documentation of treatment plans and progress notes.
- Knowledge of discharge planning processes and experience developing comprehensive care plans for varied clinical scenarios.
