Job Requirements:
- Visit and assess patients to complete an initial comprehensive OASIS or assessment and ongoing assessments through end of care.
- Chart all clinical documentation in our EHR software
- Create and manage patient plan of care
- Educate and give instruction to patient, family, and caregivers as appropriate per patient care plan.
- Case manage the patient episode or benefit period, ensuring that all cares are performed and documented
- Provides medication education and management per physician order
- Works hand-in-hand with the IDT to ensure patient goals are met and appropriate outcomes are achieved
- Shares on-call duties
- Maintains strong communication with patient, family, caregivers, and the interdisciplinary teams
Job Qualifications:
- Registered nurse with current license to practice professional nursing in the state of Utah
- Polite and compassionate
- Excellent observation, problem solving, verbal and written communications
- Maintains a current CPR certification
- 1 year Home Health or Hospice experience preferred
- Must be a licensed driver with an automobile that is insured, registered and in good working order
- Good communication and smartphone skills
- Ability to pass a background check
