Benefits:Company partiesCompetitive salaryEmployee discountsPaid time offTraining & development Job Title/Position: Scheduling CoordinatorReports To: Executive Director/AdministratorJob Description SummaryResponsible aspects of scheduling new admission of patients, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and verifying insurance and completing authorization processes.Essential Job Functions/ResponsibilitiesEnsures seamless transition of patients to home care by providing oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.Builds and monitors community and customer perceptions of Active Health Services as a high quality provider of services.Assists in strategic planning including identifying opportunities for additional or improved services to meet customer needs.Maintains comprehensive working knowledge of Active Health Services contractual relationships and ensures that patients are admitted according to contract provisions.Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Active Health Services.The above statements are intended to be a representative summary of the major duties and responsibilities performed by incumbents of this job.
