Job Summary
Home Care Direct Support Professionals (“DSP”) are responsible for providing, restoring, maintaining, and promoting the health status of our clients through home support, medical monitoring, client transportation services, and assistance with activities of daily living. Job Responsibilities:
- Providing day to day direction and supervision as well as managing performance of direct support staff to include development, training, and providing constructive feedback.
- Preparation of staff schedules, review and maintain thorough and accurate documentation, including but not limited to: HPC service documentation, behavior documentation, communication logs, incident reports, medication documentation, and mileage records
- Support and contribute to an environment of trust, sharing, growth, and mutual support.
- Maintain open communication and cooperative work efforts with supervisors and other staff to ensure quality and continuity of support for the individuals served.
- Provide opportunities throughout the day to assist clients in meeting their individual goals.
- Assist clients with all aspects of personal care, including bathing, toileting, and basic hygiene needs.
- Adhere to the person-centered plans and behavior plans as specified for each resident.
- Ensure a safe and healthy environment, free from harm, while respecting the rights of all individuals. Foster trust and build relationships with all clients and teammates.
- Prepare meals as necessary based on dietary requirements, food specifications, and documentation.
- Transport clients to and from scheduled activities or appointments, attend appointments, advocate on their behalf, and obtain necessary documentation as required.
- High school diploma or equivalent
- At least 21 years of age.
- Prior experience working with seniors and/or adults with special needs is preferred.
- For positions that involve driving for clients (i.e. errands/Dr. appointments): A valid Driver’s license, safe and reliable transportation, and proof of vehicle registration and insurance are required.
- Valid C.P.R. and First Aid certificate prior to employment preferred.
- Strong communication and engaging interpersonal skills.
- Able to work a flexible schedule including overnight if accept assignment for a 24/7 client.
- Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds; able to stand for an extended period while providing physical assistance to members.
