$50 per visit or $42 per hour

The RN or LPN will work to provide the highest quality of care to patients utilizing the Plan of Care provided to ensure patient's needs are met in relation to safety, well-being and satisfaction. Assisting patient's attending doctor's appointments, medication management visits, wellness checks.
JOB RESPONSIBILITIES
Provides safe and effective care for the client based on individualized Plan of Care.
Provides prompt communication of changes in patient medical status/emergencies to Team resulting in effective management of cases.
Provides safe and effective nursing for the patient, based on the nursing scope of practice and individualized Plan of Care in conjunction with Nurse Supervisor/Manager of Clinical Practice. May include, but not limited to, medication administration, wound care, infusion, transferring & positioning patient, catheter care, effective communication with the client, family, and Team Case Manager
performs supervisory visits.
Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.
REQUIRED EXPERIENCE & EDUCATION
- Current RN or LPN license, state of Florida
- IV Certification is preferred.
- Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
- Must be well organized and have superior time management skills.
- Must have wound care and infusion skills.
REQUIRED CREDENTIALS
- current CPR
- current negative TB (PPD within last 12 months / CXR within last 5 years)
- current Eligible status on the AHCA Clearinghouse
