- Clinic Location: 240 Commerce Dr # F, Crystal Lake, IL 60014
- Schedule: Monday - Friday, daytime hours (8am-5pm) * May volunteer for an occasional Saturday based on patient needs

- High School/Associate’s Degree: $17.00 - $23.50
- Bachelor’s Degree: $19.00 - $27.00
- Master’s Degree: $20.00 - $28.00
