What You’ll Do:

• Provide high-quality, one-on-one physical therapy to geriatric patients in their homes.
• Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
• Implement effective plans of care to improve patient mobility and safety.
• Coordinate seamlessly with our nursing and clinical leadership team.

What You Need:

• Active Florida Physical Therapist (PT) License.
• Current CPR/BLS Certification.
• Valid Driver’s License and reliable transportation.
• Home Health experience preferred, but we will train the right clinician! (Kinnser/WellSky and OASIS experience is a plus).
