đĄ What Youâll Do (and Love!)
â Champion Member Health
- Oversee all healthcare services for your members â delivering support, guidance, and clinical insight that elevates their well-being.
- Document medical needs, interventions, and wellness strategies with precision and heart.
â Be a Care Advocate
- Participate in Individual Support Plans and case conferences, ensuring every memberâs voice, needs, and goals are heard.
- Collaborate closely with Primary Care Physicians to coordinate care thatâs seamless and proactive.
â Lead With Purpose
- Manage treatment plans and ensure medical compliance across the board.
- Conduct preadmission reviews, assessments, and create thoughtful AFC plans of care.
- Recruit, train, and inspire AFC caregivers â shaping a confident, prepared care team.
â Drive Quality & Excellence
- Keep medical records confidential, updated, and organized.
- Perform high-level QA tasks, including audits and policy reviews.
- Work with external agencies to maximize support, resources, and services for members.
- Report and manage high-profile case issues with professionalism and confidence.
đ What Makes You Exceptional
Essential Skills
- Strong clinical background in patient care and nursing.
- Competent in triage, medication management, and care planning.
- Comfortable supervising and supporting adult learners.
- Familiarity with Electronic Health Records and digital documentation.
Bonus Points (But Not Required!)
- Experience with AFC, home health, or similar programs.
- EMR experience (especially Icentrix).
- Ability to train and mentor caregivers with patience and clarity.
- Comfort using remote communication tools like Zoom.
đ Requirements
- Current RN licensure from the Massachusetts Board of Registration in Nursing.
- Ideally, 1 year of supervisory experience.
- Valid driverâs license, personal vehicle, auto insurance, and annual inspection.
- Satisfactory CORI, SORI, and National Fingerprint Check.
- Basic computer and documentation skills.
