💡 What You’ll Do (and Love!)

⭐ Champion Member Health

• Oversee all healthcare services for your members — delivering support, guidance, and clinical insight that elevates their well-being.
• Document medical needs, interventions, and wellness strategies with precision and heart.

⭐ Be a Care Advocate

• Participate in Individual Support Plans and case conferences, ensuring every member’s voice, needs, and goals are heard.
• Collaborate closely with Primary Care Physicians to coordinate care that’s seamless and proactive.

⭐ Lead With Purpose

• Manage treatment plans and ensure medical compliance across the board.
• Conduct preadmission reviews, assessments, and create thoughtful AFC plans of care.
• Recruit, train, and inspire AFC caregivers — shaping a confident, prepared care team.

⭐ Drive Quality & Excellence

• Keep medical records confidential, updated, and organized.
• Perform high-level QA tasks, including audits and policy reviews.
• Work with external agencies to maximize support, resources, and services for members.
• Report and manage high-profile case issues with professionalism and confidence.

🛠 What Makes You Exceptional

Essential Skills

• Strong clinical background in patient care and nursing.
• Competent in triage, medication management, and care planning.
• Comfortable supervising and supporting adult learners.
• Familiarity with Electronic Health Records and digital documentation.

Bonus Points (But Not Required!)

• Experience with AFC, home health, or similar programs.
• EMR experience (especially Icentrix).
• Ability to train and mentor caregivers with patience and clarity.
• Comfort using remote communication tools like Zoom.

🎓 Requirements

• Current RN licensure from the Massachusetts Board of Registration in Nursing.
• Ideally, 1 year of supervisory experience.
• Valid driver’s license, personal vehicle, auto insurance, and annual inspection.
• Satisfactory CORI, SORI, and National Fingerprint Check.
• Basic computer and documentation skills.

The pay range for this position is $80000.00 - $90000.00/yr.
