Registered Nurse - Care Coordinator

Shift: Monday – Friday, 8:00 AM – 5:00 PM
Position Summary
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team. The RN will provide high-quality nursing care, coordinate patient services, and collaborate with physicians and advanced practice providers. This role involves independent decision-making within the RN scope of practice and supports our mission to deliver exceptional patient care.
Minimum Requirements
- Current California RN License
- IV certification preferred
- Proficiency with Microsoft Office and Electronic Health Records (NextGen experience preferred)
- Ability to travel between sites
- Experience in ambulatory care nursing
- Current Basic Life Support (BLS) certification
Key Responsibilities
- Develop and implement standardized nursing procedures
- Manage chronic disease care and transitional care for discharged patients
- Conduct triage and respond to emergencies
- Oversee medication administration and delegate tasks appropriately
- Provide patient education and skilled nursing care within scope of practice
- Participate in committees, outreach, and staff training
- Ensure compliance with safety, infection control, and quality standards
Skills & Abilities
- Strong knowledge of nursing theory, patient care, and medical equipment
- Ability to work in a team environment and handle multiple priorities
- Excellent communication and customer service skills
- Proficient in problem-solving and maintaining accurate records
Interview Process
- One-step panel interview via Zoom with the CNO, Site Manager, and possibly a Provider
Experience Level
Open to all levels if the candidate demonstrates strong skills and potential.
Job Type & Location
This is a Contract position based out of Stockton, CA.
Pay and Benefits
The pay range for this position is $50.00 - $53.00/hr.
