Job Title: Licensed Practical Nurse (LPN)
Job Description
We are seeking a dedicated Licensed Practical Nurse (LPN) Care Coordinator to join our team in Norfolk. The ideal candidate will efficiently manage patient visits, including chief complaint assessment, medication and allergy reconciliation, and smoking status assessment. You will also prepare exam rooms, restock clinical areas, and ensure medical equipment is sterilized and maintained. Additionally, you will respond to patient messages, handle prescription refills, lab results, and manage referrals and prior authorizations.
Responsibilities
- Conduct patient intake and care coordination.
- Handle prior authorizations and medication reviews/refills.
- Document interventions in electronic medical records.
- Review charts for completeness and obtain necessary medical records.
- Respond to patient messages and manage prescription refills and lab results.
- Manage referrals and prior authorizations.
- Collect patient satisfaction surveys and conduct follow-up assessments.
- Ensure adherence to infection control, environmental safety, and patient confidentiality protocols.
- Maintain annual recertification for OSHA, compliance, respiratory fit, and IT policy.
Essential Skills
- Practical Nurse License.
- BLS certification.
- Experience in nursing, patient care, and care management.
- Proficiency in medical records management and electronic health records.
- Experience in clinic settings, chart review, and triage.
- Strong case management and care coordination skills.
Additional Skills & Qualifications
- Ability to work in a family medicine department.
- Exposure to a variety of illnesses and treatments.
Work Environment
This role is based in a family medicine office in Norfolk. The work environment is supportive of professional growth within a large healthcare organization, providing exposure to various care settings. This is a Contract to Hire position based out of Virginia Beach, VA.
Pay and Benefits
The pay range for this position is $28.00 - $32.00/hr.
