Job Description

The Clinical Assessment Manager completes comprehensive clinical assessments and ensures accurate, timely documentation using the UAS-NY system. This role plays a critical part in both enrollment and ongoing care planning, supporting interdisciplinary teams through detailed evaluations, informed care recommendations, and high-quality clinical documentation. The position offers a highly autonomous, field-based opportunity where your clinical expertise directly impacts patient eligibility, care planning, and outcomes for a vulnerable population, particularly within a mission-driven PACE environment focused on keeping patients safely in their homes.

Responsibilities

  • Conduct comprehensive clinical assessments using the UAS-NY system to determine eligibility and support individualized care planning.
  • Complete clinical onboarding assessments, scheduled reassessments, and post-hospital discharge evaluations in a timely manner.
  • Review patient history, diagnoses, medications, and other clinical information to ensure accurate and complete documentation in the electronic medical record (EMR).
  • Document all assessments, clinical notes, and related findings within required timelines, including completion within 48 hours of each assessment.
  • Collaborate closely with intake, enrollment, and care delivery teams to support continuity of care and ensure smooth transitions across services.
  • Escalate clinical concerns, significant changes in condition, and complex cases to appropriate internal teams for further review and intervention.
  • Maintain a daily schedule of approximately 2–3 UAS assessments, managing time effectively to meet productivity and quality expectations.
  • Provide case management support as needed, integrating assessment findings into care plans and coordinating with interdisciplinary team members.
  • Utilize EMR systems and Microsoft Office tools to manage clinical documentation, track assessments, and communicate with internal stakeholders.
  • Contribute to a mission-driven care model by helping shape care plans and influencing clinical decisions that improve quality of life for elderly and frail participants.

Essential Skills

  • Active Registered Nurse (RN) license in New York State.
  • Strong working knowledge of UAS-NY, with demonstrated experience using the system (required).
  • 3–5 years of experience in MLTC, PACE, MAP, LTHHCP, or other managed care programs.
  • 5 years of experience in case management, utilization review, population health, or discharge planning.
  • Experience working with elderly or frail populations in community-based or managed care settings.
  • Ability to work independently in a field-based role while managing multiple assessments and priorities.
  • Strong documentation skills with the ability to complete accurate, timely clinical notes and assessments.
  • Effective communication skills to collaborate with intake, enrollment, and care delivery teams.
  • Strong organizational skills to maintain a daily schedule of 2–3 UAS assessments and meet documentation timelines.
  • Comfort and proficiency using electronic medical record (EMR) systems.
  • Proficiency with Microsoft Office applications for documentation and communication.
  • Reliable transportation and willingness to travel throughout the Bronx and Westchester areas.

Additional Skills & Qualifications

  • Bachelor of Science in Nursing (BSN) or related healthcare degree preferred.
  • Experience in PACE programs and other managed long-term care models is highly valued.
  • Background in MLTC, PACE, MAP, LTHHCP, or similar managed care environments.
  • Demonstrated ability to manage multiple assessments in a fast-paced environment while maintaining quality and compliance.
  • Strong attention to detail when reviewing medical records, diagnoses, and medication lists.
  • Ability to identify and escalate significant changes in condition or complex clinical issues to internal teams.
  • Commitment to serving vulnerable, elderly, and frail populations and supporting them to remain safely in their homes.
  • Comfort working in a highly autonomous role where clinical judgment and initiative are essential.

Work Environment

This is a highly autonomous, field-based clinical role within a mission-driven organization focused on keeping participants safely in their homes through comprehensive, coordinated care. The schedule is Monday through Friday, 8:30 AM to 5:30 PM, with a 1-hour lunch. The position involves regular travel throughout the Bronx and Westchester areas to complete in-person UAS-NY assessments and post-hospital discharge evaluations. You will work closely with interdisciplinary teams while spending much of your time in the field and documenting in an electronic medical record (EMR) system. The role requires regular use of EMR platforms and Microsoft Office applications to manage clinical documentation, communication, and scheduling in a fast-paced environment.

This is a Contract to Hire position based out of Bronx, NY.

Pay and Benefits

The pay range for this position is $60.00 - $65.00/hr.
