Description
Provide comprehensive primary care to a panel of medically complex senior patients
Take full accountability for patient outcomes, including admissions, quality metrics, and patient experience
Lead and coach an interdisciplinary care team (medical assistants, care coordinators, facilitators)
Serve as a clinical leader and mentor to other primary care physicians
Facilitate weekly care team huddles and performance discussions
Partner with operational leadership to improve clinical quality, efficiency, and access
Engage with hospitalists and transitional care teams when patients are hospitalized
Contribute to long‑term center growth, culture, and performance
Additional Skills & Qualifications
Required:
MD or DO
Board‑certified or board‑eligible in Internal Medicine or Family Medicine
Active or eligible medical license
Commitment to caring for senior populations
Preferred:
Experience in value‑based or risk‑bearing care models
Prior physician leadership or mentoring experience
Strong interest in clinical outcomes, analytics, and team‑based care
