Job Description
- The incumbent will work as part of an interdisciplinary team in the serving individuals with severe and persistent mental illness including those with co-occurring disorders.
- Incumbent will act as a Care Manager in performing assessments.
- Helping develop individualized recovery plans, determining appropriate level of care, placing individuals in those levels of care and then ensuring that care is delivered according to standards.
- Other duties will include visiting beneficiaries in the community, charting, and acting as liaison with a large network of community providers.
