Community Liaison – Behavioral Health Services (Nursing Homes & Assisted Living Facilities)
Position Overview
The Community Liaison serves as the primary outreach and relationship-building representative for the organization, focusing on establishing and maintaining partnerships with nursing homes, assisted living facilities, rehabilitation centers, and other senior-care providers. This role is responsible for increasing awareness of the company’s behavioral health services and generating new client referrals through strategic marketing, education, and communication.
Key Responsibilities
Outreach & Relationship Building
- Develop and maintain strong, ongoing relationships with administrators, social workers, directors of nursing, case managers, and other referral sources within long-term care and senior-living communities.
- Schedule and conduct on-site visits, presentations, and educational sessions on behavioral health offerings and their benefits for elderly residents.
- Serve as the point of contact for partner facilities, ensuring positive communication and prompt follow-up to maintain trust and satisfaction.
Marketing & Business Development
- Implement outreach strategies to increase brand visibility and generate new client referrals.
- Identify new partnership opportunities and expand the referral network within assigned territories.
- Represent the organization at community events, health fairs, professional networking groups, and industry conferences.
Education & Advocacy
- Provide informational materials and training to facility staff on behavioral health services, Medicare/insurance processes, and appropriate referral procedures.
- Promote awareness of mental health challenges common among elderly populations, including depression, anxiety, cognitive decline, and adjustment disorders.
Coordination & Collaboration
- Work closely with internal clinical and administrative teams to ensure referred clients are scheduled promptly and services are delivered smoothly.
- Track referral activity, outreach efforts, and pipeline updates using CRM systems or internal reporting tools.
- Provide feedback from community partners to improve service quality and client satisfaction.
Compliance & Professionalism
- Uphold all HIPAA guidelines, ethical standards, and organizational policies.
- Maintain professionalism, cultural sensitivity, and respect when interacting with vulnerable populations and partner facilities.
Qualifications
Required
- Strong interpersonal, communication, and presentation skills.
- Ability to build trust and maintain long-term professional relationships.
- Valid driver’s license and ability to travel to multiple facilities.
Preferred
- Experience in healthcare marketing, community outreach, business development, or related field.
- Knowledge of long-term care environments (SNFs, ALFs, rehabilitation centers, etc.).
- Experience specifically in behavioral health or geriatric care.
- Existing relationships with nursing homes or senior-living providers.
- Background in social work, psychology, healthcare management, or related fields.
- An Associate degree or Bachelor degree in a health care related field.
Pay: $16.00 - $22.00 per hour
Expected hours: 16.0 – 24.0 per week
