Community Health & Outreach Specialist (On-site, full-time)

Job Description
The Community Health & Outreach Specialist will lead community engagement efforts, coordinate our Promotoras (community health volunteers), and deliver culturally responsive outreach and education initiatives. This role is well-suited for someone who enjoys working directly with the community, building strong relationships, and helping individuals access information and services that support their well-being. The position is primarily focused on outreach, education, and community engagement, with client support provided as needed.
Role and Responsibilities
- Build relationships and connect community members to services and resources.
- Conduct outreach in neighborhoods, events, and community spaces across Montgomery County.
- Coordinate and implement community education on topics impacting the community, including health, wellness, environmental issues (e.g., lead), and civic engagement (e.g., Census).
- Develop and share culturally relevant educational materials and outreach content.
- Lead and support community workshops (e.g., Healthy Fridays), including preparation of materials and basic evaluation tools.
- Promote health screenings, wellness practices, and healthy behaviors using culturally competent approaches.
- Coordinate on-site health screenings (e.g., mammograms, prostate exams, cervical cancer, insulin, and blood pressure screenings) in partnership with healthcare providers.
- Provide basic navigation support on healthcare systems, insurance options, and available resources.
- Represent ACLAMO in community meetings, coalitions, and local initiatives, and advocate for equitable access to services.
- Lead and coordinate ACLAMO’s Promotoras de la Salud program, including recruitment, training, and ongoing support.
- Collaborate with promotoras to implement grassroots outreach and deliver culturally appropriate education.
- Support immunization education and outreach efforts and assist with vaccination initiatives as needed.
- Assist individuals with access to benefits and services and provide referrals to reduce barriers to care.
Qualifications & Education Requirements
- Experience in community outreach, social services, or community health
- Strong interpersonal skills and ability to engage effectively with diverse communities
- Strong communication and facilitation skills (written and verbal)
- Ability to build trust and work respectfully with community members where they are
- Ability to lead, organize, and motivate volunteers
- Bilingual (English/Spanish)
- Organized, flexible, and able to manage multiple priorities and deadlines
- Strong attention to detail and ability to maintain accurate records
- Ability to work both independently and as part of a team
- Understanding of the community’s needs and commitment to serving diverse and underserved populations
- Commitment to confidentiality, privacy, and dignity of all participants
- Passion for ACLAMO’s mission, vision, and programs
Preferred Skills
- Proficiency in Microsoft Excel, Word, Outlook, and PowerPoint
- Experience with data entry, tracking, and reporting using case management or CRM systems (e.g.,Apricot)
- Ability to manage and track client data using standardized systems and processes
Special Notes
- Must have and maintain a valid drivers’ license, automobile insurance and daily access to an automobile and cell phone.
- This position requires to work Monday thru Fridays, some nights and/or weekends.
- This position requires visual and auditory acuity and the physical ability to ascending or descending ladders, stairs, ramps among others. Adjusting, lifting up, carrying up or moving objects up to 35 pounds in all directions. Standing or sitting for prolonged periods, walk and work in your feet for a minimun of 6 hours at a time.
- This position requieres traveling to all ACLAMO offices.
- Must obtain and maintain the following clearances:
Pennsylvania Child Abuse
Pennsylvania State Police Criminal Record Check; and
Federal Bureau of Investigation Criminal Background Check.
